You've Already Done
the Hardest Part.
Deciding to Reach Out.
Fill out the form below and we will be in touch within one business day, usually sooner. If you have questions before submitting, you are welcome to call or email directly. There is no wrong way to start this conversation.

If you're sitting on this page wondering whether what you're dealing with is "bad enough" to reach out: that uncertainty is itself a reason to reach out.
Most of the families and individuals who contact us are not at the beginning of their struggle. Many have already tried something: a course of well-meaning talk therapy, a program that produced partial improvement, months or years of weekly sessions that circled the same ground without breaking through. People do not arrive here because they haven't tried. They arrive because what they've tried hasn't been enough.
If that sounds familiar, we'd love to hear from you. An inquiry is not a commitment to anything. It's a brief conversation to figure out whether we're the right fit, and if we're not, we'll tell you and do our best to help you find who is.
A note on how we deliver care: our clinicians work both in person and via secure telehealth. We have in-person clinicians in Boca Raton, Houston, Chicago, and Denver, with in-person caregiver mentorship in Austin, and most of our doctoral team is PSYPACT-credentialed for telehealth across 40+ states (with added coverage in New York and Hawaii). During your first conversation, we'll match you with the right clinician and the format that fits your situation best.
Concerned about paying out of pocket?
We're a private-pay practice, which keeps your care driven by clinical need rather than an insurer's rules. That doesn't mean you're on your own with the cost. Here's what most families use:
- Out-of-network reimbursement. We provide a monthly superbill you can submit to your insurer; many plans reimburse a portion of each session.
- HSA & FSA funds. Sessions are typically eligible, so you can pay with pre-tax health-spending dollars.
- Full transparency. Our fees are published up front. Nothing is hidden, and no decision is required before our first conversation.
What Happens After You Submit
A clinician reads your inquiry, usually the same day
Your inquiry doesn't sit in a queue or get triaged by a scheduler. A real clinician reads what you've shared, thinks about who on our team is the best fit for your specific situation, and reaches out personally.
We schedule a brief screening call
We'll contact you by phone or email, whichever you prefer, to set up a short conversation. Usually 20 to 30 minutes. No paperwork, no forms, no pressure. Just a real conversation.
The screening call itself
You tell us what's been going on. We listen, ask questions, and answer yours honestly. If we're a good clinical fit, we discuss next steps and scheduling. If we're not, we'll say so directly, and help you figure out who is. Most people leave this call feeling clearer than they expected to.
Your first session
If we move forward, we schedule your intake, an extended first session that goes beyond assessment to start the actual clinical work. Most patients are seen within five to seven days of their initial inquiry.
Prefer to Reach Out Directly?
Filling out forms isn't for everyone. If you'd rather pick up the phone or send a quick email, please do. Both options reach the same clinical team and we'll respond just as quickly.
Anything you share in this form is read only by the Evidence-Based Treatment Institute's clinical team and is kept fully confidential. Submitting an inquiry doesn't create a patient-provider relationship and doesn't obligate you to schedule or begin treatment in any way. If you're in a psychiatric emergency right now, please call 988 or go to your nearest emergency room, and reach out to us when you're safe and stable.
A Few Places That Might Help.
Not sure if we treat what you're dealing with?
We treat eating disorders, OCD, anxiety, trauma, women's mental health, ODD, and longstanding behavioral and emotional patterns. If you're not sure whether your situation fits, reach out anyway; the worst outcome is a helpful conversation.
See All Conditions We TreatWondering whether private pay is worth it?
We explain our fee structure completely, including how out-of-network reimbursement works, what Superbills are, and how to use HSA and FSA funds. Nothing is hidden and no decision is required before our first conversation.
